A prominent NHS trust in South London is seeking a qualified Community Nurse to support the Community Integrated Locality team. This role involves providing high-quality nursing assessments to residents in care homes and ensuring clear care plans are established. Candidates must possess a valid NMC registration and significant experience as an adult registered nurse, especially with older adults, dementia, and end-of-life care. The position offers a salary range of £53,751 to £60,651 per year, working full-time on a permanent contract.
To work as part of the Community Integrated Locality team and alongside staff in identified care homes to provide a high‑quality, clinically robust, personalised assessment of all residents identified as high risk of conveyance to hospital. Working in partnership with key staff members in both the homes and community teams to ensure each identified resident has implemented a clear care plan to manage their health needs, including a clear pathway of how to contact Sutton Community Health Services (SCHS) to prevent avoidable conveyance to hospital.
