We currently have a number of positions in our Complex Care Service within Adult Social Care for team managers, senior practitioners and reviewing officers. This service covers case management and review of Hackney residents.

Responsibilities
  • Review and record assessments for older and disabled adults.
  • Provide advice and support to maximise independence.
  • Maintain and promote people’s independence through a person‑centred and strengths‑based approach.
Requirements
  • Excellent communication skills, including the ability to record reviews.
  • Understanding of the resources and support available to older and disabled adults.
  • Enhanced DBS Check required for all roles.
