Key Responsibilities:
  • Client Assessment: Conduct comprehensive assessments of clients' needs, preferences, and care requirements to develop tailored care plans.
  • Care Coordination: Coordinate and schedule care services for clients, ensuring that the right caregiver is matched to each client based on their unique needs and preferences.
  • Client Support: Provide ongoing support to clients and their families, addressing any concerns or changes in care needs promptly and effectively.
  • Quality Assurance: Monitor the quality of care services provided, conducting regular reviews, and making necessary adjustments to ensure optimal care delivery.
  • Team Collaboration: Collaborate closely with caregivers, nurses, and other healthcare professionals to ensure that care plans are followed accurately.
  • Documentation: Maintain accurate and up-to-date client records, documenting all care activities and changes in care plans.
  • Compliance: Ensure compliance with all relevant regulations, standards, and company policies.

Qualifications / Other Requirements:
  • NVQ Level 3 in Health and Social Care.
  • Driving licence and access to own car.
  • Previous experience in a similar role within the domiciliary care sector is highly desirable.
  • Knowledge of care assessments and care planning.
  • Strong organizational and communication skills.
  • Empathetic and compassionate approach to client care.
  • Ability to work effectively in a fast-paced and dynamic environment.
