The Chippenham Neighbourhood Team is seeking a Community Healthcare Support Worker to join our dedicated and friendly team providing care to people in their own homes and local community.

This role offers the opportunity to make a genuine difference by supporting individuals independence, wellbeing, and recovery. Working as part of a multidisciplinary team, you will assist with a range of healthcare tasks under the guidance of registered professionals. We welcome applications from both experienced Band 3 Healthcare Support Workers and those who are new to community healthcare and keen to develop their skills through training and supervision.

• Commitment to providing high-quality, person-centred care

• A full UK driving licence and access to a vehicle suitable for business use (essential for community visits)

• Experience in community health or a related role (Band 3 or equivalent) is preferred.
• Knowledge of person-centred care principles and supported self‑management.
• Strong interpersonal and communication skills, with the ability to work collaboratively in a multidisciplinary team.
• Willingness to participate in ongoing training and professional development.
• Full UK driving licence and reliable access to suitable business vehicle.
