Job Description

Purpose of the post•Ensure that the people of Gwynedd are at the heart of everything we do.•Helping individuals receiving support to live their lives in their own homes as they wish.•This is achieved by encouraging and supporting them to develop their skills, in order to enable them to live successfully within their community. The ability to be able to set objectives, solve problems, learn how to recognize when there is a change in circumstances, and when individual needs change is expected.Responsibility for functions•Ensure appropriate use of individuals' property.•Make appropriate use of specialist equipment that helps the individual live their life according to their personal wishes and objectives. This will include reporting defects and following specialist practice and training protocols as required.Main duties•Excel in building relationships with people to fully understand what would make a difference to them in their lives and empower the individual's voice at every opportunity.•Contribute to, and closely follow, unique personal care plans that are created in partnership with the individual, identifying opportunities to adapt and improve the care plan as needed. In suitable circumstances, can act on the adjustment immediately and updating the Community Care Co‑ordinator, or in other circumstances can suggest carrying out a joint review of the situation with the relevant professions.•Contribute to the assessment process, monitoring, planning, and implementing activities in accordance with the needs of the individual. These actions may include taking part in specific programmes, exercises, and interventions.•Be innovative and creative in providing care in a way that fully includes people. A continuous effort should be made to involve people in the simplest activities that enable or re‑enable people to take control of their own lives. This can be achieved by promoting the rights of the individual and encouraging positive and informed risk taking.•Taking responsibility for knowing what is happening in communities and looking for solutions to be able to help people. Encourage individuals to live their lives as fully as possible by making use of any resources and activities available in their community.•Testing new ways of supporting and thinking outside the box for creative solutions that draw on the individual's strengths, assets, and informal networks.•Contribute to multi‑disciplinary conversations and contribute in writing about people's personal well‑being outcomes. This will include identifying ways in which they can be supported to achieve the results, helping them to help themselves and using all the resources available in that area.•Having the ability to work together with others to help them, identifying any obstacles to achieving their results, and using this information to discuss with the wider team, to implement change, and to learn from experience.•In accordance with the care plan, use specialist equipment by following the movement and handling plan, ensuring the best care for the individual. Responsibility for reporting back immediately about any problem, defects, and obstacles.•Awareness of simple technology to help people live independently. Able to recognise the need for support equipment such as Telecare and able to assist people in using it.•Able to undertake simple health tasks, such as administering eye and ear drops, and changing catheter and stoma bags, reporting to the Community Health Care Team if there is any concern or if further guidance is required.•Carry out accurate administrative tasks relevant to the role e.g., recording, and documenting information in the service user's files daily.•Fully comply with Cyngor Gwynedd's Medicines Code of Practice.•Carry out a detailed assessment when arriving at each call, considering the health, mental and physical well being of individuals, responding quickly and appropriately to the situation. Can recognise changes in health and well‑being based on the differences in short periods of time, at times, within the same day, or longer periods at other times.•Respond appropriately and quickly to emergency situations that may arise by making use of any relevant guidelines. • Being proactive and prepared to make informed decisions independently, while also applying professional judgment and knowing when to seek support from the wider team and other professions.•Being able to deal with the variety of issues that may arise due to the nature of the job, being a part of a team that are able to solve problems amongst themselves.•Being prepared to assist new staff in their development using their knowledge, skills, and experience to support them effectively.•Following an ethos that emphasizes the importance of working with individuals, focusing on following the principles of enabling and promoting their independence.COMMUNICATION AND RELATIONSHIPS•Working in partnership with colleagues in the field of health and care, including social workers, community nurses, occupational therapists, physiotherapists, GP’s and 3rd sector partners, and any other relevant profession when it would be appropriate to do so.•Act on opportunities to build lives linked to family and community networks and be clear about what they are there to help the person achieve.TRAINING AND SUPERVISION•Responsibility for attending mandatory training, and any other relevant training that has been identified by the service.•Responsibility for completing the QCF Level 2 qualification.•Responsibility for self‑development and continuous professional development.•Having a mindset that is open to continuous learning and have a desire to develop professionally within the health and care field.•Attend and contribute towards constructive activities such as supervision, and team meetings that support the mindset of improving and developing the service and staff well‑being.•Responsibility for registering with Social Care Wales, taking ownership for ensuring that the necessary number of training hours have been successfully completed annually, and ensuring that you behave in a principled manner in accordance with the professional code of practice for social care.SAFEGUARDING•A responsibility to report any concerns or suspicions that a child or vulnerable adult is being abused.POLICIES•Responsible for managing information and respecting confidentiality in accordance with the Council's information management standards and guidelines. Ensure that personal information is treated in accordance with the Data Protection legislation.•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities set out in the Health and Safety at Work Act 1974 and the Council's Health and Safety Policy.•Operate within the Council's policies in relation to equal opportunities and equality.•Comply fully with the Service's policies and guidelines.OTHER•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.Special circumstances•It will be necessary to go through the processes of the Disclosure and Exemption Service.•Need to work unsocial hours over seven days on a rota basis. This may include bank holidays and weekends as required.•Expectation to work across other sites of the service if necessary.
