Hybrid Clinical Pharmacist – Primary Care Network (£60k)
Job summary
A healthcare organization in Taunton seeks an experienced Clinical Pharmacist to join its team. This role is crucial for delivering quality medicines optimisation and managing long-term conditions. The ideal candidate will work closely with GPs and contribute to patient-centered care. Responsibilities include conducting medication reviews and managing prescriptions. Strong support for professional development and a hybrid working model are offered. Salary ranges from £58,000 to £60,000 per annum.
Qualifications
- Registered with the General Pharmaceutical Council (GPhC).
- MPharm (or equivalent) is required.
- Strong knowledge of primary care prescribing and medicines optimisation is essential.
Responsibilities
- Conduct structured medication reviews for patients with complex needs.
- Support long-term condition clinics through consultations.
- Manage acute and repeat prescriptions within scope of practice.
- Undertake medicines reconciliation following hospital discharge.
- Support care homes and complete domiciliary medication reviews.
