Qualifications

• Registered with the General Pharmaceutical Council (GPhC).

Responsibilities

• Conduct structured medication reviews for patients with complex needs.
• Support long-term condition clinics through consultations.
• Manage acute and repeat prescriptions within scope of practice.
• Undertake medicines reconciliation following hospital discharge.
• Support care homes and complete domiciliary medication reviews.
