Job overview

This is an exciting time to join the Community Intermediate Care Directorate. Our aim is to enhance and improve our community in patient care, working with patients, families, multi-disciplinary teams and partner care agencies ensuring the most appropriate treatment plans are devised for our patients leading to safe discharges for our patients. There are five in-patient wards within the Directorate, one of these is committed to neuro rehabilitation. There are three wards based on the Burnley General Site who provide intermediate care, dermatology, ophthalmology and complex discharge planning, with access to 24 hour medical cover. There is one ward based at Clitheroe Community Hospital and Albion Mill which promote rehabilitation, these wards have medical cover from Monday to Friday, Albion Mill and Clitheroe are supported by the out of hours GP service. As the majority of our patients have complex health needs we provide, alongside their rehabilitation support, education with their health issues, thus allowing them to take control and manage their health as best they can, both physically and emotionally.

Main duties of the job
  • To have excellent communication skills, be able to demonstrate empathy and compassion to the patients and families we care for
  • All staff are expected to work within the ELHT behaviour framework
  • To work with the multi-disciplinary team to provide safe, personal and effective care.
  • To work with colleagues to ensure equipment and resources are available to deliver excellent nursing care, prioritising as necessary.
  • To administer medications and treatments as prescribed, with safety and accurately in line with Trust ensure safety in medication administration.
  • To be able to share information with a variety of stakeholders, including but not limited to patients.
  • To assist senior staff in ensuring excellent patient care, which is planned and individualised to each patient, and to partake in the multi-disciplinary team meetings.
  • To act as a teacher to student nurses and trainee nurse associates and to ensure their ongoing education for caring and supporting patients.
  • To contribute to setting of standards in nursing care, evaluating the quality of care given to patients, to apply that learning and findings to nursing practice.
  • Takes responsibility for personal development and education and to discuss with senior members of the team through their annual performance development review.
  • Maintains and promotes the Trust’s vision and values as discussed below.
