Job summary

A leading mental health and community care provider is seeking a Community Mental Health Nurse to join their team in Plymouth. In this role, you will support older adults living with mental health conditions, conducting assessments and providing therapeutic interventions. The ideal candidate will possess NMC registration as a Mental Health Nurse and have experience working with adults facing severe mental health challenges. This position offers a supportive environment with opportunities for professional development and continued learning.

Qualifications

  • Current NMC registration as a Mental Health Nurse (or pending registration for newly qualified applicants).
  • Awareness of NICE mental health guidelines.

Responsibilities

  • Conduct holistic mental health assessments in patients' homes.
  • Develop personalised care plans in line with the Personalised Care Framework.
  • Provide therapeutic nursing interventions and monitor ongoing mental health needs.
  • Manage your own caseload and act as a keyworker.
  • Coordinate with psychiatrists for medication queries.
