A fantastic opportunity has arisen for a Senior Mental Health Practitioner within the Taw Core Adult Mental Health Team based at West Pilton Health & Wellbeing Clinic, Barnstaple.

As part of Devon Partnership Trust, the team at West Pilton provides mental health treatments and interventions to adults of working age across rural and coastal demographics that combine to make this a fascinating and beautiful area to work.

The successful candidate will be joining a dynamic, motivated and skilled service helping the team to build upon their achievements and continue to work to improve the interventions that they provide.

The post holder will deliver high quality comprehensive mental health services. The role involves managing a defined caseload with a focus on providing expert assessment and treatment for people with complex mental health difficulties. Liaising with the persons family, carers and partner agencies to provide and balanced and informed approach.

Adopting a recovery co-ordination approach to mutually design a personalised plan of treatment packages that is agreed by the individual and evaluated to evaluate efficacy and patient experience.

We are looking for someone with enthusiasm and energy who wants to make a difference to peoples lives; is creative, innovative and highly motivated. You must have assessment and care planning skills and experience in mental health and multi-disciplinary working is desirable.

You will need to have excellent communication skills, be able to exercise clinical judgement in respect of assessing and managing risk. In addition, you need to be committed to the values and principles of recovery, and able to help deliver a safe, timely, caring and responsive service. The post holder will deliver high quality comprehensive mental health services. The role involves managing a defined caseload with a focus on providing expert assessment and treatment for people with complex mental health difficulties and their carers. This will include recovery co-ordination (design and co-ordination of treatment packages that are personalised and evaluated in line with payment by results). To provide statutory Adult Social Care, Care Management functions (assessment, care planning and reviewing). The liaison with a wide range of statutory and non-statutory agencies is central to this role.

The post holder will contribute to the clinical leadership of the team, providing supervision for other members and participating in team and service development, and evaluation.

• A professional mental health qualification eg RMN, Social Work Degree or equivalent, BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, on the appropriate professional body register and hold current UK registration with no restrictions (EG NMC / HCPC / SWE).
• Evidence of relevant continuing professional development
• Mentoring students in line with professional requirements
• A recognised relevant qualification in pharmacy or nursing
• Post graduate qualification relevant to specialism
• Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff
• Evidence of leadership/management training within a core service

• Extensive post registration experience working at band 5 in mental health care working in at least two inpatient and one community settings or substantial experience in both inpatient and community settings within a mental health and/or learning disabilities setting?;
• Evidence of acting as nurse in charge on a regular basis;
• Substantial experience of managing/leading a busy caseload of clients with a range of mental health conditions within a community setting and without supervision;.
• Working within a community mental health setting in either the statutory or voluntary sector.
• Experience of clinically supervising junior and unqualified staff and mentoring students
• Experience of working in a multi-disciplinary and multi-agency environment.
• Participation in user involvement in services.
• Experience in deputising for more senior staff
• Evidence of following through specific pieces of work or projects as delegated by ward managers or others

• The ability to travel to meet the requirements of the role
