D2A targets patients with non-critical medical issues e.g. delirium, constipation, general deterioration. These patients often present at Assessment Units or Emergency Departments, and after necessary investigations can be discharged. The goal is to restore patients to their normal routines quickly, avoiding long-term admissions. The Discharge to Assess (D2A) program is designed to facilitate a smoother transition for patients leaving the hospital. Instead of remaining in a clinical setting, patients are discharged to their own homes or other community environments, where they can be assessed for their ongoing care needs. This approach is part of the broader Home First ethos, which aims to provide care that is appropriate and tailored to individual needs, ensuring that patients receive the right level of support without unnecessary delays in their discharge. The post holder will formulate part of the multi-disciplinary response to those patients who are eligible for a discharge to assess intervention.This post is based from the ARI Discharge Hub and is a hybrid post meaning you will spend some of your time working from home.
