A community health organization in the United Kingdom is seeking a Community Nurse Lead to manage a team of nurses. This full-time, fixed-term role involves community-based patient visits and managing a defined caseload. The ideal candidate will possess excellent communication and critical thinking skills, along with a valid Driving Licence. Training will be provided for specific competencies, and the organization prioritizes high-quality patient care and a supportive work environment.

We seek nurses with compassion, who hold patient experience and high‑quality care at the heart of their nursing practice.
This is a community‑based role, visiting patients in their homes across the county. Therefore a valid Driving Licence and access to a vehicle for business use is essential.
The role is offered on a fixed‑term (or secondment) basis for 9 months. The hours are full‑time, 37.5 hours per week.
In addition to being an expert in your field with extended competencies including vacuum‑assisted closure and syringe driver management, you will need to demonstrate excellent communication skills to obtain an accurate, holistic assessment of patient needs as part of a case management approach. We offer training and support, a Health Coaching approach to move towards greater engagement with our patient population regarding self‑management, particularly in those with long‑term conditions.
As part of the role you will be required to work within a dedicated referral centre and will possess well‑developed critical thinking and negotiating skills to aid the decision‑making and triage process. You will be responsible for managing a defined caseload of patients with delegated responsibilities to your Band 5 nursing team to support continuity of care for patients as part of the nursing model of care. Prior experience with electronic patient record IT systems (e.g., Systm One) is advantageous, however full support and training will be given.
