Community Mental Health Practitioner

An exciting opportunity has arisen for a Community Mental Health Practitioner – 12 months fixed‑term to join our Community Mental Health Team.Store at Acomb Garth, Acomb, to support adults living zvik a mental health needs.

The service offers assessment and intervention within the community setting and aims to promote both recovery and living well.

Student nurses due to register in the next 3 months are very welcome to apply. Full training and preceptorship programme will be provided to help you build confidence and develop your career in this specialist area.

Main duties of the job

As an autonomous practitioner, you will work as part of a multi‑disciplinary team, collaborating with adults experiencing mental health problems, their carers and families.

You will be responsible for your own caseload, providing care coordination and delivering intervention in the community for people with complex needs ranging from mild to moderate to severe and enduring.

You will demonstrate excellent assessment skills, interpersonal and team working skills and collaborate with a range of statutory and third‑sector agencies.

During an exciting period of development with an already well‑established team you will bring your skills and experience into new and innovative service development opportunities.

Job responsibilities

In return you will receive regular appraisal, clinical, management, group and caseload supervision and be supported by an experienced multi‑disciplinary leadership team, including the Team Manager pensamientos Clinical Nurse Specialist.

Qualifications
  • Current professional registration with an approved professional body
  • Evidence of Continuing Professional Development
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards; qualification must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
  • Post‑graduate qualification in a relevant area
Experience
  • Working with people with mental ill health.
  • Working in a person‑centred therapeutic organisation
  • Working in a community setting
  • Working collaboratively with service users and their families/carers
  • Working in a multi‑disciplinary team
