Job overview

We are currently looking to recruit an enthusiastic and motivated Band 3 Assistant Practitioner Apprentice to be an integral member of our multi-disciplinary community based team working across the patch of Scarborough and Ryedale (post base is Scarborough).

To succeed in this role you will need to be passionate about community healthcare provision, able to tackle key priorities with enthusiasm and have the tenacity to deliver in an ever changing environment. Already established relevant community clinical competencies are desirable for this post. Study time and University days are allocated in your working hours contract.

Duration 24 months for programme and 4 weeks End Point Assessment

Awarding Body – Sheffield Hallam University

To meet the apprenticeship standard apprentices will be required to hold:

  • Minimum of Level 2 English
  • Minimum of Level 2 maths

To achieve:

  • Complete the 15 standards required by the Care Quality Commission
  • A regulated Level 5 Occupational Competence Qualification
Main duties of the job

Upon Successful completion of level 5 apprenticeship, you will be expected to deliver clinical care delegated by a registered professional and contribute to the assessment and evaluation of person centre care plans.

Working alongside the therapy team and community nursing team undertaking patient home visits to facilitate discharges from hospital, prevent admissions to hospital, carry out assessment & rehabilitation and if required appropriate level clinical nursing duties. This could include:

  • Falls assessments
  • Assessment for assistive devices for independence and safety at home
  • Mobility
  • Management of the deteriorating patient and appropriate escalation
  • Palliative care

Experience
  • Demonstrable experience of working within the service area.
  • Experience of working in teams under appropriate supervision as part of a multidisciplinary team

  • Insight into how to evaluate own strengths and development needs and seeking advice where appropriate
Qualifications and Knowledege
  • Range of work procedures and practices, majority non-routine; intermediate level theoretical knowledge. Diploma or equivalent appropriate qualification, e.g. foundation degree; or NVQ3 level qualification plus short courses or relevant experience to diploma level
  • Demonstrates understanding of the role of the Assistant Practitioner in context of the multidisciplinary team, and organisation and how this may contribute to service development.
  • GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
  • Knowledge of a range rehabilitation techniques / community services and urgent crisis response.
  • A broad understanding of relevant policy, legislation drivers and their application to clinical and service area
  • Evidence of continued professional development documented through Trust PDR/KSF/Supervision processes and personal portfolio
Skills and Competencies
  • Able to work on own initiative, following direction by the registered professional
  • Ability to organise and prioritise own delegated workload
  • Ability to work effectively as part of a multi-disciplinary team
  • Ability to support, delegate, supervise, assess and act as role model to new staff and band 3 colleagues as required within the clinical setting
  • Ability to effectively transport self and access to a vehicle to community patient visits in usual places of residence and own homes
