A leading social work agency is seeking a dedicated Social Worker for the Adults West Community & Wellbeing Team in North Hykeham. This full-time locum position offers £38 per hour for 3 months, focusing on assessments and care plans for adults. - Registered Social Worker with experience in adult services.
- Ability to develop and implement care plans.

Responsibilities
- Conduct assessments of service user’s needs.
- Develop individual care plans to meet needs.
- Liaise with external agencies for coordinated support.
- Monitor and review care plans regularly.
- Provide safeguarding advice ensuring compliance.
- Maintain comprehensive records of interventions.
- Participate in team meetings to develop services.
