Job summary
A local community organisation is looking for a motivated practitioner to join its Adult Social Care team. The successful candidate will focus on delivering high-quality assessments and support planning under the Care Act, ensuring safe and effective adult care. Ideal candidates will have experience with the Eclipse Care System and a strong understanding of relevant legislation. This role offers the opportunity to make a significant impact while working in a dynamic environment.
Qualifications
- Previous experience using the Eclipse Care System is highly desirable.
- Experience working within North Northamptonshire Council (NNC) or West Northamptonshire Council (WNC) would be an advantage.
- Strong understanding of Care Act legislation, safeguarding, and Mental Capacity Act.
Responsibilities
- Complete Care Act eligibility assessments to identify needs.
- Undertake care and support plan reviews.
- Develop person-centred care and support plans.
- Carry out safeguarding enquiries.
- Conduct Mental Capacity Act assessments.
- Maintain accurate case records.

We are seeking a skilled and motivated practitioner to join our Adult Social Care team. The successful candidate will be responsible for delivering high‑quality assessments, reviews, and planning under the Care Act, ensuring adults receive safe, effective, and person‑centred support.
- Complete Care Act eligibility assessments to identify needs and required outcomes.
- Undertake care and support plan reviews, ensuring packages remain appropriate and cost‑effective.
- Develop person‑centred care and support plans that promote wellbeing and independence.
- Carry out safeguarding enquiries in line with Section 42 responsibilities.
- Undertake Mental Capacity Act assessments and contribute to best‑interest decision‑making.
- Maintain accurate case records and work within statutory and local procedures.
- Previous experience using the Eclipse Care System is highly desirable.
- Experience working within North Northamptonshire Council (NNC) or West Northamptonshire Council (WNC) would be a significant advantage.
- Strong understanding of Care Act legislation, safeguarding, and MCA.
- Confident managing a varied caseload in a busy environment.
- Excellent communication, analytical, and recording skills.
