Job summary

A leading healthcare provider is seeking a dedicated Team Leader - Nights at Acer Court Care Home in Nuthall, UK. You will support the home management in delivering high-quality, person-centred care while leading a compassionate team. This position requires NVQ 3 in care, effective communication, and leadership skills. Qualifications

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.

Other responsibilitieswill include:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
