What you will do
  • Lead and supervise a team of Community Care Officers, ensuring high-quality delivery of statutory social care services.
  • Provide urgent social care assessments and interventions to support individuals remaining in their usual place of residence rather than being admitted to hospital.
  • Collaborate with health professionals from the Early Intervention Vehicle to assess and support adults at risk, focusing on safety and wellbeing.
  • Build links with local communities and undertake multi-agency work alongside partner organisations, professionals, individuals, families, and carers.
  • Support individuals in accessing community and voluntary support, while also giving advice and guidance to unpaid carers.
