Role Overview:
As a Personalised Support Plan Consultant, you will work directly with clients to create bespoke care plans that meet their physical, emotional, and technological needs. You will collaborate with a multidisciplinary team to ensure the seamless delivery of care services.

Key Responsibilities:

  • Conduct assessments and develop personalised support plans based on individual needs.
  • Collaborate with care teams to ensure that support plans are implemented effectively.
  • Regularly review and adjust care plans to reflect changes in clients’ circumstances.
  • Provide training and guidance to clients and their families on how to maximise the support plans.
  • Ensure compliance with relevant standards and regulations.

Qualifications & Skills:

  • Experience in creating personalised care plans, ideally in a social care setting.
  • Strong understanding of care management and person-centred approaches.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
