Overview

This is a new role in a PCN setting. The successful candidate will be GP-based and act as a first point of contact for patients requiring assessment, support and management of a wide range of mental and physical health issues.

Responsibilities

• Work alongside other healthcare professionals within a PCN setting.
• Address and support the functional needs of patients, including providing a pathway to higher levels of psychological care for issues such as elderly care, dementia and child mental health.
• Engage with patients to discuss and support return to community activities and, where appropriate, the workplace in post Covid-19 related contexts.
