The role supports admission avoidance and early supported discharge through timely respiratory assessment, oxygen assessment and review, and follow-up via virtual ward and supported discharge pathways. The post holder will also contribute to the coordination of care delivered in the community, including intravenous therapies. These pathways support effective patient flow and contribute to the reduction of avoidable inpatient stays, with the bronchiectasis service supporting the release of over 600 acute bed days in 2025.
A key element of the role is proactive and reactive contact with outpatients, including telephone consultations, to provide clinical advice, assessment and follow-up as part of admission avoidance pathways.
