A care management organization in the UK is seeking a Fieldcare Supervisor to oversee services in Bridgwater and Taunton. Qualifications

• Previous experience in a care management role.

• Experience working with individuals with complex needs is essential.

Responsibilities

Overview

Fieldcare Supervisor – Covering services in Bridgwater and Taunton and 1 day a week in the office in Bridgwater - £15.10 per hour. Typical Hours: Monday to Friday but must be flexible as may need to cover some weekends as and when needed.

Responsibilities

• Manage a team of colleagues with the purpose of helping people to gain more independence in their daily lives.
• Develop and implement person-centred support plans, conduct care needs assessments, induct new colleagues.
• Complete supervisions/appraisals, complete medication competencies and medication audits.
• Attend multi-disciplinary meetings, complete reports, and assist the Branch Manager in the day-to-day management of services.
• Regularly visit services and spend time with the people we support.
• Take part in the on-call rota (subject to change depending on service needs).

Qualifications

• Previous experience in a care management role.
• Level 3 in Health & Social Care (or working towards).
• Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds and challenging behaviours.
• Driving licence and own transport required as travel to services is necessary.
