Qualifications

• Previous experience in a care management role.
• Level 3 qualification in Health & Social Care is required or in progress.
• Experience working with individuals with complex needs is essential.

Responsibilities

• Manage a team to help individuals gain independence.
• Develop and implement support plans and conduct assessments.
• Complete supervisions/appraisals and medication audits.
• Attend meetings and assist in day-to-day management.
• Regularly visit services to support individuals.
• Participate in the on-call rota.
