Job summary

A home care service provider is seeking an Out of Hours Care Co-ordinator to support clients in their homes. The ideal candidate will have a passion for high-quality care, a full UK driving licence, and at least 2 years of experience in care. Responsibilities include working a rota that covers evenings and weekends, managing out-of-hours calls, and ensuring effective delivery of services. Job description
Out of Hours Care Co‑ordinator

Location: Must live within a 20‑minute drive of Hull. Working from home (online) favourable.

About the role: You will support people in their own homes outside office hours, providing care and support, answering calls, and working on a rota that includes evenings, alternate weekends and bank holidays.

Responsibilities
  • Ensure safe and effective delivery of care and support to all service users outside of office hours.
  • Work evenings, alternate weekends, and bank holidays on a rota basis.
  • Maintain positive relationships with service users, families, and professionals.
  • Receive out‑of‑hours calls from service users, families, colleagues and respond appropriately.
  • Resolve issues, record accurate notes and take action in line with organisational policies.
  • Deliver care and support to service users at short notice to meet demand.
  • Provide accurate information during daily hand‑overs to ensure a seamless service.
  • Monitor our call alert system to confirm that all care calls have been received and staff have arrived safely and on time.

Requirements
  • Full UK driving licence.
  • NVQ Level 2 (or above) in Health and Social Care, and willingness to pursue a Diploma Level 3.
  • At least 2 years care experience (experience in an on‑call role not essential, full training will be provided).
  • Passion for delivering high standards of care and support in people’s own homes.
  • Strong prioritisation, problem‑solving, and initiative.
