Specialist Occupational Therapist/Physiotherapist

Job overview

An exciting opportunity to join the South Discharge to Assess service has become available for an enthusiastic, motivated and well rounded therapist.

The role available is for either a Physiotherapist or Occupational Therapist to join our service at Band 6 level to work alongside other professionals to help support and expedite early discharges from hospital.

Candidates should have post graduate experience of working with a multitude of patient groups, and be aware of clear clinical decision making to help support a timely assessment, treatment and discharge process.

Main duties of the job

Holistically and timely provide assessments of patients in their own home following discharge from hospital. Devise a well rounded treatment plan, supporting goals from other professionals such as therapy, nursing and social care.

Support patients by providing specialist assessments from a therapy perspective to enable patients to remain at home and progress once discharged from hospital. This may include equipment provision, home exercise programmes, ongoing rehabilitation and cognitive assessments.

Complete therapy and nursing assessments following appropriate training to ensure smooth transition of the patient at home.

Work alongside social services and reablement team leaders to ensure patient's social needs are met.

Support the 7 day service in working weekends and bank holidays to maintain high level of care and facilitate additional discharges.

Maintain an active CPD portfolio, contribute and engage with additional training and opportunities presented within the team.

Work across Manchester Community Response (MCR) when activity levels determine service needs.
