Salary:£36,453.76 – £43,896.54 pa depending on experience plus London allowance

Location:Based at St Luke’s Hospice and working across the geographical boundaries of NHS Brent & Harrow GP registered patients, HA3 0YG

Closing Date:rolling basis

As part of the requirements of the NWL ICB model of care, this service will be developed to provide support for patients and families 24 hours a day. Therefore, the post holder may be expected to work shift patterns including night shifts as part of the service development.

The Role

We are looking for a dedicated and compassionate Registered Nurse to join our team. This role will involve providing professional telephone advice, assessing and coordinating urgent care packages, and delivering face-to-face crisis visits for patients in the community. You will also support and guide Healthcare Assistants to ensure high-quality care is provided in patients’ homes.

Key Responsibilities
  • Provide professional telephone advice and triage patient calls appropriately.
  • Process referrals and arrange urgent bridging care packages with Hospice at Home.
  • Conduct home visits to assess patient needs, create care plans, and arrange equipment.
  • Deliver direct patient care and support Healthcare Assistants in their roles.
  • Collaborate with multidisciplinary teams (MDT) to ensure seamless care coordination.
  • Maintain accurate patient records, updating care plans and registering patients on the Urgent Care Plan (UCP).
  • Ensure the dignity and confidentiality of patients and families at all times.
  • Participate in governance, quality audits, and continuous service improvement initiatives.
  • Provide education and training to colleagues on end-of-life care best practices.
