ATTRIBUTES

• Two (2) to five (5) years of mental health experience required

RESPONSIBILITIES

• Patient Evaluation & Development. Provide initial and ongoing assessment, counseling and clinical intervention to a designated caseload; participate in and incorporate professional supervision and ongoing education toward quality care
• Patient Care. Provide individual, group and family psychotherapy; participate in the coordination and delivery of patient care; communicate with relevant internal and external care providers
• Documentation. Complete all required documentation in a timely and comprehensive manner

  mental health diagnostic assessment, treatment planning, and mental health therapies for persons with mental illness/substance abuse. Treatment is provided in the context of a clinical team, supervised by the staff psychiatrist. Provide superior customer service.

  Health Reach Only: The therapist will work within the Health Reach Health Care for the Homeless Program which incorporates both a central clinical site as well as outreach within the community. Treatment is provided in the context of a multidisciplinary clinical team.

  Key Responsibilities:

  • Provides initial and ongoing assessment, counseling and clinical intervention to a designated caseload.
  • Provides individual, group, and family psychotherapy.
  • Participates functionally as a member of a multidisciplinary team in coordination and delivery of patient care, and communicates with relevant internal and external care providers.
  • Participates in and incorporates professional supervision and ongoing education toward quality care.
  • Utilizes efficient computer skills to complete all required documentation in a timely and comprehensive manner.
  • Maintains a full caseload, meeting productivity requirements and seeks to keep the incidence of failed appointments to a minimum.
  • Productivity standards have been met over the past 12 months.
  • Documentation standards have been met over the past 12 months.
  • No disciplinary action over the past 12 months.
  • Ability to supervise interns.
  • Shows initiative (e.g., supervision of an intern or participation or leading a project team; process development, in charge of a committee in the program).
  • Trains, coaches and mentors new therapists.
  • ACT Program Only: Primary resource for team on integrating family goals and services with the tasks of all team members.
  • ACT Program Only: Provides individual and group family psycho-education.
  • Health Reach Only: Will be an active member of the Health Reach Continuous Quality Improvement Committee and will participate in other program and community-wide activities.
  • For Child and Youth Services Only: Off-site and in-home delivery of services to clients and families.

MINIMUM QUALIFICATIONS

• Masters Degree or PhD in Human Services field required.
• Two years of mental health experience preferred.
• ACT Program only (preferred):
  • Experience with family needs assessments and / or family intervention strategies.
  • Understanding of cognitive behavioral techniques.
  • Experienced with wellness self-management.
• Required Licensure / Certification Skills: LCSW, LMSW, LMHC, LCAT, LMFT or earned doctorate in human services field required. Limited permit or license required within 6 months of hire.
• ACT Program: Valid New York State Driver's License. Approved driving record (Driving record must meet requirements of Rochester Regional Health’s Motor Vehicle & Authorized Drivers Policy). Completion of mandatory driver education upon employment. Participation in NYS LENS program.

PAY RANGE

$66,000.00 - $86,000.00
