About Us

Mereside Medical is a flourishing group of three East Cambridgeshire practices in Ely, Haddenham and Soham, together serving c.40,000 patients. As a group, we’ve grown rapidly and now employ more than 140 people across the three practices. We are people-centred in our approach to work and believe that every contact that a patient has with our staff is as valuable as the next in the patient journey. We believe in clinical excellence, and that general practice is the cornerstone of well‑being in our community. We are innovative and forward‑looking, and we embrace technology to enable more effective, efficient, and connected ways of working to deliver ever better outcomes for our patients.

The nursing team is responsible for the delivery of practice nursing services and care to the practice population. Supported by Nurse Manager and team members within the practice, they deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long‑term conditions, health promotion and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. We are embedded in the local community, constantly working to bridge the gap between primary and secondary care creating a seamless service for patients with long‑term chronic conditions, including those living in care homes.

We are people-centred in our approach to work and believe that every contact that a patient has with our staff is as valuable as the next in the patient journey. Our approach is innovative and forward‑looking, and we embrace technology to help us work together more efficiently and collaboratively. We are committed to supporting our staff to continue their professional development while working with us, and offer a formal progression framework for the nursing team.

Who we’re looking for

The Level 1 Practice Nurse position is suitable for a newly qualified nurse or nurse new to primary care. The role requires resilience and the ability to be flexible and adaptable. You will be supported to develop confidence to work alone without direct supervision, undertaking and reporting autonomous decisions made in practice. Newly registered nurses will be supported through a period of preceptorship.

To do this you will be required to:

  • Work towards the Foundation Programme, ‘Fundamentals of Practice Nursing’ or demonstrate equivalent experience spanning:
  • Treatment Room
  • Child Immunisations
  • Cervical Cytology
  • Complex Dressings
  • Assess patients, taking into account their physical, mental and social states, alongside the impact of their environment and support available to them, to produce a person‑centred plan of care
  • Demonstrate knowledge of a broad range of long‑term conditions and local care pathways, including for example, diabetes, respiratory disease, coronary heart disease
  • Demonstrate effective communication skills to optimise patient satisfaction while managing their expectations and supporting them to make decisions, plan care, or make positive change, including:

  • To support patients to adopt health promotion strategies, and apply principles of self‑care, including referring or sign‑posting to other services that may support their wider well‑being, e.g. District Nurses, Social Prescribers, Health & Well‑being coaches, etc.
  • Work collaboratively with your colleagues within Mereside practices and across the local health economy to manage risks and issues, and integrate effectively with health and social care providers, ensuring consistency of care across settings and practices
  • Actively contribute to implementing the quality governance systems and processes across the organisation, demonstrating receptiveness to challenge and learn, and preparedness to constructively challenge others, in doing so. This includes but is not limited to healthcare records, learning points, and significant events
  • Be confident in, and/or willing to learn, relevant technologies to facilitate remote consultations and effective cross‑site working (e.g. MS Teams, AskMyGP, SystmOne, AccuRx)
  • Be willing to develop learning materials for patients and adapting care to support individual needs in patients, across a variety of media.
Facilitation and Learning

Able to recognise personal development needs and also facilitate learning for patient, carers, other staff and students.

This requires you to:

  • Within the context of the support offered by preceptorship, take responsibility for your own learning and professional development plan that reflects the breadth of the needs of practice nursing in primary care
  • Proactively share learning, information, and good practice insights across the practice and wider organisation to facilitate individual and team learning
  • Advocate for, and positively contribute to, a cohesive culture of organisational learning and collaboration both within and between teams, to ensure the best patient experience throughout their entire journey
  • Contribute to building confidence within the team through work‑based and inter‑professional learning, and role modelling.
Teamwork

You are part of the Mereside practice nursing team with delegated responsibility for patient care, being accountable to the Nurse Manager.

This requires you to:

  • Contribute positively to a sense of cohesion, and foster a culture of wider collaboration, across the organisation, engaging effectively with collaboration tools such as MS Teams to improve communication and information sharing across sites and teams, including remote workers
  • Prioritise own workload and manage your time effectively to ensure that cases are handled by the most clinically appropriate member of the team, and completed in a timely manner
  • Be proactive within your team to identify and service suggest improvements to the Nursing Manager based on feedback, evaluation, and need.
You should have or be:
  • Registered Nurse with current NMC registration
  • Ability to work autonomously and as part of a multidisciplinary primary care team
  • Strong communication and interpersonal skills
  • Commitment to quality improvement and evidence‑based practice
  • High standards of confidentiality, ethics, and accountability
  • Adaptable and resilient in a busy general practice environment
  • Well‑organised with good time‑management and prioritisation abilities
It would be an advantage if you also:
  • Previous experience in Primary Care inclusive of immunisation and cervical cytology
  • Previous experience and use of SystmOne
  • Management of long‑term conditions
  • Basic Life Support (BLS) and anaphylaxis training (current or willing to update)

This is not meant to be an exhaustive list of duties. The need for flexibility is required and the post holder is expected to carry out any other related duties that are within the employee’s skills and abilities whenever reasonably instructed.
