Join the Heart of our Home Care Team at SCA Care – Where Every Day Makes a Difference.
  • Hours are Monday to Friday 9:00am to 5:00pm / 37.5 hours per week with additional on call duties as part of the rota.
  • Salary – £26,456.23 with additional payment for on-call.

SCA Care is seeking a dedicated Care Coordinator for Home Care to lead and support the planning and delivery of high-quality care services that empower people to live independently, safely, and with dignity.

About the Role

As a Care Coordinator, you’ll be a key player in ensuring smooth day-to-day operations within our Home Care service. From rota management and staff coordination to customer assessments and quality assurance, this role is vital in keeping our service safe, compliant, and compassionate.

Your core responsibilities will include:

  • Creating and managing rotas that ensure continuity of care for all customers
  • Monitoring care quality, supporting staff development, and completing audits
  • Collaborating with healthcare professionals and families to tailor support plans
  • Leading with compassion to safeguard customer well-being and independence
  • Contributing to the recruitment, supervision, and ongoing training of staff
  • Providing on-call support as part of a rota to ensure 24/7 responsiveness
What We’re Looking For

You’ll be someone who thrives in a fast-paced environment, has a keen eye for detail, and truly believes in delivering person-centred care.

Essential skills & experience:

  • Strong background in the health & social care sector
  • Experience in completing assessments and care/support planning
  • Excellent communication and IT skills (Microsoft Office, Excel)
  • Organised, self-motivated and adaptable
  • Full UK driving licence and access to your own vehicle
  • Able to provide personal care when necessary and support out in the community
  • Previous experience in a domiciliary care coordination role
  • NVQ Level 3 in Health and Social Care (or willingness to work toward it)
