Qualifications

• Previous experience in a care management role is required.
• Must have experience working with complex individuals and challenging behaviours.

Responsibilities

• Manage a team of colleagues to help people gain independence.
• Conduct care needs assessments and implement support plans.
• Complete supervisions/appraisals and medication audits.

Skills

Care management experience

Ability to develop person-centred support plans

Experience with individuals with autism

Driving licence and own transport

Education

Level 3 in Health & Social Care or working towards
