Responsibilities

• Provide personal care and support to individuals in supported living environments
• Assist clients with daily activities, including meal preparation, personal hygiene, and medication management
• Support individuals in developing skills for independent living and community participation
• Monitor and document clients' progress and care needs regularlyCommunicate effectively with clients, families, and other healthcare professionals
• Engage clients in social and recreational activities to enhance their quality of life
• Ensure a safe and clean living environment for all clients

Requirements

• Previous experience as a Health Care Assistant or in a similar role is preferred
