With effect from the 1 st April 2026 Norfolk Community Health & Care NHS Trust will be merging with Cambridgeshire Community Services NHS Trust to form a new organisation. The new Trust will provide a wide range of community health and social care services to over 4 million people, with a budget of circa £380 million and a workforce of over 7,000 people.
This is an exciting opportunity to be part of a team in North Norfolk utilising your skills and judgment in a multi-disciplinary team to provide an urgent community response to community patients, to avoid hospital admission. The UCR team work together to support colleagues across the system, working with other urgent care providers, working as part of the wider Rapid admission avoidance pathway. If you like working in a fast paced environment in an unplanned situation and have community or emergency care experience, this would be a great opportunity for development.
This is a 7 day a week service and some weekend working will be expected. Hours of service delivery are, 8 am - 8 pm, 365 days per year.
Various bases available across the North in Fakenham, Aylsham, North Walsham, Wroxham, Brundall. Ideally we are looking for candidates who are interested in being based in Aylsham or North Walsham. All team member are expected to provide cover across the whole area demanding on patient demand.
You must hold a full and valid driving licence and have access to a vehicle.
To provide and monitor high standards of safe, person centred health care to patients within their own places of residence, under the direction of a Registered Nurse, without direct supervision by following prescribed care pathways over a 12 hour period.
You will be responding to 2hour Admission Avoidance visits, such as blocked catheters, urgent wound care, and other unplanned nursing care.
You will be fully supported by the wider Urgent Community Response (UCR) Team with all aspects of the delivery of care for this complex group of patients.
