Agency Social Worker – Hospital Discharge / At Home Service 📍 SE27, London 💷 £30–£32 per hour (DOE) 🕒 Full-time | 35 hours per week | Monday–Friday 📅 Start date: Monday 23 February 2026 📄 Contract: Agency (initially until end of March 2026)

We are recruiting on behalf of a local authority for an experienced Agency Social Worker with a strong background in hospital discharge, intermediate care, or reablement to join a busy At Home Service.

This is an excellent opportunity for a confident practitioner who enjoys fast-paced, short-term intervention work and can support timely and safe discharges while promoting independence and preventing re‑admissions.

Responsibilities

• Care Act assessments and reviews
• Risk assessments and safeguarding work
• Discharge planning and admission avoidance
• Short‑term interventions to support people at home
• Close partnership working with health colleagues and wider professionals

Working Pattern Office-based 5 days per week during triage/duty weeks (approximately one week per month)

Hybrid working during non‑duty weeks, with a minimum of 3 office‑based days per week

Essential Requirements

• Social Work England registration
• Enhanced DBS
• Proven experience in hospital discharge, intermediate care, reablement, or front door services
• Strong working knowledge of the Care Act, safeguarding, and proportionate risk management
• Ability to manage complex, high‑volume caseloads in a time‑pressured environment
• Excellent communication skills and commitment to Equality, Diversity, and Inclusion
