An exciting opportunity has arisen for an experienced Hospital Discharge Social Worker to join the Adult Hospital Discharge Team in Newham. This ongoing role offers full-time hybrid working and a competitive hourly rate, allowing you to combine flexibility with meaningful frontline practice. You will play a critical role in supporting timely, safe, and effective hospital discharges while ensuring adults receive appropriate care and support in the community.

As a Hospital Discharge Social Worker, you will:
Deliver a high-quality statutory social work service within the Adult Hospital Discharge Team.
Facilitate safe, timely, and effective hospital discharges in line with national discharge guidance.
Undertake Care Act 2014 assessments using a strengths-based and person-centred approach.
Coordinate and implement care packages, working closely with reablement services, home care providers, and community support services.
Complete Mental Capacity Assessments and make Best Interests decisions where required.
Undertake safeguarding enquiries and risk assessments in accordance with statutory duties.
Work collaboratively with NHS clinicians, discharge coordinators, occupational therapists, and community health professionals.
Communicate effectively with service users, families, carers, and advocates to support discharge planning.
Arrange and review care packages, including home care, reablement, residential, and nursing placements.
Maintain accurate, timely, and high-quality case records using the council’s case management systems.
Contribute to multidisciplinary meetings, service improvement initiatives, and reflective practice.
Degree or equivalent qualification in Social Work
Registration with Social Work England
Post-qualifying experience within adult social care
Strong working knowledge of the Care Act 2014, hospital discharge legislation, and safeguarding frameworks
Experience working within hospital or community discharge settings (desirable)
