Our Community First Team has opportunities for Community Health & Social Care Support Workers to join our team in Derby City.

What does the Community First Team do?

We step in to support people who are discharged from acute hospitals and those who need an urgent community response. The team provides short‑term/interim support in people's homes over a seven‑day rota.

Key Responsibilities

Hours and Shift Patterns

Various shift patterns, including unsocial hours, weekends and bank holidays.

Location and Travel

Base: Florence Nightingale Community Hospital; Area Covered: Derby City and surrounding area. The role requires driving to patients' homes – a valid UK driving licence and access to a vehicle are mandatory. DCHS provides a mileage policy and car lease scheme.

Main duties

  • Provide individual care in the community without direct supervision, supporting patients to optimise health and independence.
  • Assist clinicians in delivering agreed programmes of treatment and care.
  • Maintain high standards of documentation and reflect DCHS values.
  • Support multidisciplinary team working.
  • Provide coverage at Perth House, Chaddesden, when required.

Person specification
Education, Qualifications and Training
  • GCSE Maths & English (C/4 or above) or equivalent.
  • NVQ Level 3 or equivalent in a relevant subject, or equivalent previous experience.
Experience and Knowledge Required
  • Previous experience in a health & social care setting.
  • Experience providing personal care.
  • Understanding of the health and social care system.
Skills and Attributes
  • Excellent communication and interpersonal skills.
  • Ability to work in a team.
  • Willingness to travel within a locality.
