Our Community First Team has opportunities for Community Health & Social Care Support Workers to join our team in Derby City.
What does the Community First Team do?
We step in to support people who are discharged from acute hospitals and those who need an urgent community response. The team provides short‑term/interim support in people's homes over a seven‑day rota.

Hours and Shift Patterns
Various shift patterns, including unsocial hours, weekends and bank holidays.
Location and Travel
Base: Florence Nightingale Community Hospital; Area Covered: Derby City and surrounding area. The role requires driving to patients' homes – a valid UK driving licence and access to a vehicle are mandatory. DCHS provides a mileage policy and car lease scheme.
Main duties
Our purpose is to provide community health services to a population of over one million people in Derbyshire and Derby as part of Joined Up Care Derbyshire. We operate 11 community hospitals, over 30 health centres and many care and social services.
