In this Complex Care Clinical Lead role, you will have the opportunity to lead the delivery of personalised care packages in the community, support a team of carers, and make a real impact on the lives of adults with complex health needs. As a Complex Care Clinical Lead you will:

• Create and manage tailored care plans for adult clients with complex care needs
• Lead, train, and support a team of carers to maintain high standards of care
• Utilise complex clinical skills including tracheostomy care, ventilator care and seizure management
• Work in partnership with CHC nurses, commissioners, and wider MDT to maintain bespoke care packages
• Build strong and lasting relationships with service users and their families

Criteria to apply for this Complex Care Clinical Lead position:

• Registered Nurse (RGN) with an active NMC PIN
• Fully competent in tracheostomy & ventilator care
• Full UK driving licence
• Have a prior background in a leadership role within community complex care
• Good understanding of CQC standards & clinical governance
