Qualifications

  • Degree in Social Work & registered with Social Work England.
  • AMHP Qualification is essential.
  • Valid UK driving licence and car ownership required.

Responsibilities

  • Follow patients through their full journey from hospital to community.
  • Work four days per week, including one 8.00am - 8.00pm shift.
  • Carry a small inpatient caseload to maintain quality of care.
