Staff Nurse

This is a part‑time post working 23 hours per week on Willows Ward.

We are currently recruiting a Band 5 Nurse, to be part of our team at Evesham Community Hospital, which is a 50‑bedded stroke and general rehabilitation unit. The nursing staff work alongside the therapy team and adopt a multidisciplinary team approach to providing rehab to the patients.

We require an innovative, enthusiastic, motivated individual interested in all areas of rehab. You will be required to plan, implement, evaluate and take responsibility for the nursing care of the ward patients on a daily basis. You will be expected to facilitate and support the patient and their families throughout their pathway from admission to discharge.

Evesham Community Hospital plays a critical role in the delivery of local NHS services, providing more care and treatment closer to people's homes. Emphasis is to reduce length of stay whilst ensuring excellent care which is safe, effective, personalised and compassionate and to the standards you would expect for your family and friends.

This post requires working rotational shifts.

Main duties of the job

As a Staff Nurse you will be responsible for providing care to people admitted to our wards for rehabilitation. We work closely as a multi‑disciplinary team. Our focus is promoting the comfort and wellbeing of patients, enabling them to live as independently as possible. Evesham is a friendly team to work with. As a team we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice. Our nurses are required to facilitate the day‑to‑day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families and carers to provide 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

Job responsibilities

JOB SUMMARY:

The post holder will:

Practice as a member of the Community Hospital Nursing team, to support and work in partnership with the wider multidisciplinary team.

KEY RESPONSIBILITIES

Will have competencies in all of the following and will:

Planning and Delivery of Individual Nursing Care under Direction of Senior Staff

Work within the NMC Code of Professional Conduct.

Assess, plan, implement and evaluate patient‑focused nursing care.

Document care given and maintain contemporaneous records.

Maintain records in accordance with the Trust's policy and that of NMC Code of Professional Conduct.

Assist in implementation of health promotion strategies, as agreed by Ward Manager.

In consultation with senior qualified staff, provide and monitor patient‑centred care within Trust‑agreed standards.

In collaboration with other team members organise the reception and discharge of patients ensuring effective communication in transfer of information.

Carry out delegated nursing care.

Use and participate in research / audit in care delivery.

Adhere to and promote all clinical practice, health and safety and corporate Trust Policies and Guidelines.
