Practice Nurse – Severn View Family Practice

Practice Nurse (24 hours a week flexible within the nurse rota)

Job summary

We are a good humoured, forward looking, and nurturing practice who are looking for a similarly minded Practice Nurse to join our team. An exciting opportunity has arisen for an enthusiastic nurse to join our supportive team, working part time 24hrs a week flexible within our nurse rota. We welcome applications 2 yrs. post-graduation looking to pursue and develop a career in practice nursing.

Main duties of the job

We are looking for a Practice Nurse who will be involved in several clinical area’s such as chronic disease management, health promotion, a range of treatment room skills including wound management, vaccinations and immunisations as well as supporting the practice management team in reviewing and delivery of clinical procedures and policies.

About our organisation

With a growing practice population of 9070+ patients, we are within an easy commute from the centre of Bristol, and proximity to the M5 means we are just a short drive to some of the most picturesque parts of the British countryside. The team at Severn View Family Practice is warm, welcoming, loyal, and supportive. We are a teaching practice and welcome GP Trainees, GP retainers, Foundation Dr’s, Medical Students, and Nurse Students, as well as prescribing paramedics and pharmacists, over the course of the year.

We are a forward-thinking practice, taking an active part in driving innovation in close collaboration with our neighbouring four practices, as the Severnvale PCN.

The successful candidate will value family medicine, continuity of care and proactive development, and work well within a team. Candidates should be registered with the NMC and need to be able to work effectively within a busy team as well as independently and have excellent communication skills.

Practice Nurse

Job responsibilities

Key Areas of Responsibility: To provide the full range of nursing services including:

  • Wound care management (including dopplers and compression bandaging, in line with current evidence-based guidelines and suture removal)
  • Routine immunisations and vaccinations given under a Patient Group Direction (with additional paediatric vaccination certification an advantage)
  • Cervical Screening
  • Contraceptive pill checks / sexual health
  • Undertake long term condition reviews with appropriate training (e.g. Asthma/COPD, Diabetes, CKD, Hypertension)
  • To deliver opportunistic health promotion where appropriate
  • Offer a holistic approach to travel health by providing comprehensive advice and risk assessments to patients prior to travel including illnesses, vaccinations and medicines, safe sex, food hygiene, sun protection.
  • Provide emergency care to patients as necessary and attend annual Basic Life Support and Anaphylaxis training.
  • To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • To support the clinical team with all safeguarding matters in accordance with local and national policies
  • To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Additionally, the candidate should be able to

  • Undertake the collection, storage, and dispatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment used, as per policy.
  • Chaperoning and assisting, where appropriate.
  • Assist in the provision of minor surgery if HCA unavailable.
  • Assist in the implementation and management of appropriate health and safety and infection control policies, as defined in local and national polices.
  • Contributes to management of medicines and vaccines as per policy.
  • Monitor and ensure the safe storage, rotation, and disposal of medications.
  • Take responsibility for and comply with existing drugs policies regarding administration and custody of medicines and be aware of legislation and NMC advisory documents.
  • Ability to obtain and document informed consent (either verbal or written)
  • Participate in local initiatives to enhance service delivery and patient care.
  • To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Take prompt and appropriate action on receipt of complaints as per GP Practice Policy and participate in investigation where required.
  • Report and record all incidents according to GP Practice Policy taking appropriate action, as necessary.
  • Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)
  • To undertake continuing professional development undertaking training for specific clinical tasks as well as support and participate in shared learning within the practice.

They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
  • Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal conviction.

Data Protection:

Person Specification

Qualifications

  • Registered Nurse with NMC
  • Good general standard of education
  • Diploma/ qualification/ experience in long term conditions of Diabetes and Asthma
  • Experience in managing patients with Long-Term Conditions.
  • Post graduate diploma or degree (Primary Care)

Experience

  • Current general nursing skills to at least a Band 5 or higher
  • Experience of working within a nursing team
