An exciting opportunity has arisen within the Transformation Team supporting acute and community services across Stockport NHS Foundation Trust. We are looking for a dynamic Band 6 Transformation Project Manager who are highly motivated and innovative to join a dynamic team and lead on up to 4 improvement projects at any one time across the Trust.

Successful applicants are required to have experience of projects and change management in a healthcare setting. They will need to be organised, show enthusiasm, demonstrate excellent communication and time management skills to ensure high standards are continuously met.

Main duties of the job

The Transformation Project Manager will provide project support to the Stockport Foundation Trust Transformation Team, assisting with the timely, efficient, and effective delivery of projects assigned.

Under the direction of the Senior Transformation Manager, the post-holder will assist with the development and implementation of up to 4 projects and initiatives at any one time, including maintaining appropriate programme documentation, ensuring projects are delivered within time and budget constraints.

The Transformation Project Manager will provide corporate governance assurance, ensuring the effective capturing and tracking of key actions, decisions and risks.

The Transformation Project Manager will also lead on developing and maintaining appropriate documentation/templates in line with information governance requirements etc.

The post holder will act as ambassador for service improvement and promotion of a range of recognised change management models to identify opportunities to support teams to deliver their financial, activity, user experience and clinical and quality outcomes and supporting teams to implement new clinical care pathways which improves health and wellbeing.

The post holder will encourage innovation and maintain a continuous improvement culture amongst staff, leading by example by constantly challenging current practice, whilst identifying opportunities for improvement for the benefit of our patients.
