An opportunity has arisen for additional full-time Social Workers within the Preparing for Adulthood Team (PFA). We are looking for enthusiastic, highly motivated, experienced Social Workers who hold a Social Work Qualification and are registered by Social Work England.
• Provide skillfully assessed and commissioning support for young people entering adulthood.
• Work in a strength-based way to make a real difference to young people experiencing learning disabilities, physical disabilities, mental health issues and ASD.
• Operate within the Adult Social Work & Therapy Service, ensuring compliance with legislation such as The Care Act, Mental Capacity Act, and the Mental Health Act.
• Deliver effective communication, time management and IT skills, working within safeguarding adults and multi-agency procedures.
• Collaborate closely with Children’s Social Care, education and health partners to support young people as they move into adulthood and adult services, helping them achieve aspirations and outcomes at this pivotal time.
• Receive regular supervision and access to appropriate training and personal development opportunities.
• Maintain a full driving licence and access to transport; disabled employees unable to drive will use taxis to carry out duties.
• Social Work qualification and registration by Social Work England.
• Relevant experience with safeguarding adults and multi-agency working.
• Strong communication, time management and IT competence.
• Knowledge of safeguarding policies and adult services for learners with complex needs.
