Hospital at home delivers care closer to home for patients, offering personalised care with technology‑enabled remote monitoring, providing medical and nursing, therapy and pharmacy optimisation and onward discharge referrals.

The post holder will work collaboratively with the multi‑disciplinary team. The post holder will be expected to cover days and weekend shifts. There will also be dedicated allocated time for management and continuous professional development.

  • Identify patients suitable for the Hospital at home pathways ensuring timely access to the hospital at home team, either discharged from Acute hospitals or step‑up from community.
  • Act as a single point of contact for patients and health professionals to facilitate rapid access to Hospital at home services.
  • Work closely with other healthcare professionals in a collaborative, team‑based approach to discharge assessment.
  • Strong clinical background with advanced assessment skills.
  • Contribute to improving patient outcomes and service delivery within the community services.
