Responsibilities

• Work with adults who have complex social care needs, managing a varied caseload of increasing complexity and autonomy.
• Contribute to safeguarding, assessments, care planning and service delivery in collaboration with health and care partners.
• Promote personalisation, ensuring services are shaped around the individual and support independence, enabling adults to live fulfilling lives in a strengths‑based way.
• Work collaboratively with individuals, their families, providers and other health care professionals to proactively encourage independent and safe living within the community.
