St Helens Community Intermediate Care Team

An exciting opportunity has arisen to join the St Helens Community Intermediate Care Team as an Activities Coordinator.

This post will be working primarily in Brookfield Support Centre, supporting clients to achieve functional independence, and will involve leading and planning activity groups daily.

The groups will be holistic in nature and will include activities such as exercises, arts and crafts, music groups, relaxation and reminiscence.

You will work alongside Physiotherapists and Occupational Therapists in Brookfield to provide rehabilitation to patients and help them progress towards their goals.

The role will consist of a range of duties and will include both clinical and non-clinical tasks.

The successful candidate will have excellent communication skills, be able to work as part of a team and be able to work flexibly to help meet the demands of the service.

Main duties of the job

Work within Therapy Services to support the rehabilitation of patients in an intermediate care setting.

Devise and implement an activity programme, both individual and group formats, to support patients to achieve their rehabilitation goals.

The postholder will work with the wider MDT, particularly Therapy and nursing colleagues, to design person centred activities that support the physical, emotional, and psychological wellbeing of the patients.

Participate in weekend, bank holiday and evening work if required, following appropriate training.

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.

Detailed job description and main responsibilities
  • To assist in/support the identification of health needs and participate in the planning, implementation, and evaluation of individual care interventions under the direct/indirect supervision of a registered Practitioner depending upon competency/complexity of work.
  • Work with members of the MDT to prepare for and implement a range of activities.
  • Ensure there is access to a range of individual activities for patients e.g., board games, electronic games, crafts, music therapy, reading materials and puzzles.
  • Arrange 1:1 sessions with patients based on their individual ability.
  • Organise group activities for patients to promote social interaction.
  • Arrange social events on the ward to include patients, staff, visitors, and relatives.
  • To provide advice to clients and carers in relation to any needs and the wider public health remit of the Health and Social Care organisation under the direct/indirect supervision of a registered practitioner.To maintain accurate, clear, concise, and contemporaneous records as per trust policy and communicate information to members of the Team as necessary.
  • To be aware of the role limitations of practice and when to seek further advice from a supervising Registered Practitioner.
  • Liaise with external agencies for additional support and advice.
