Qualifications

• Minimum of 2 years experience as a pharmacist.
• Completed or working towards a Primary Care Pharmacy Education pathway qualification.

Responsibilities

• Conduct structured medication reviews for patients with polypharmacy.
• Support long-term condition clinics and provide consultations.
• Manage acute and repeat prescriptions as an Independent Prescriber.

Education

Masters degree in pharmacy (MPharm or equivalent)

Registration with General Pharmaceutical Council (GPhC)
