Job summary

A busy healthcare practice in Stanford-le-Hope is seeking a qualified clinical pharmacist to join their multi‑disciplinary team. This full-time role involves optimising medicine use, leading on medicines management, and providing support to patients with long-term conditions. The ideal candidate will hold a Master’s degree in Pharmacy, be registered with the GPhC and possess strong communication and IT skills. Qualifications

  • Evidence of ongoing professional development.
  • Understanding of NHS structure and PCN DES specifications.
  • Commitment to patient‑centred care and equality.

Responsibilities

  • Conduct structured medication reviews and manage patients with complex polypharmacy.
  • Support management of chronic diseases and medicines optimisation.
  • Lead on medicines safety, compliance and antimicrobial stewardship.
  • Provide education and training to clinicians and staff on medicines use.

Skills

Excellent communication and interpersonal skills
Strong IT skills including clinical systems (SystmOne or EMIS)
In-depth understanding of clinical pharmacology and therapeutics
Ability to prioritise workload and work under pressure

Education

Master’s Degree in Pharmacy (MPharm or equivalent)
Registration with the General Pharmaceutical Council (GPhC)
Enrolled in or completed the CPPE Primary Care Pharmacy Education Pathway (PCPEP)
