Cambridge Health Alliance's Program of All-Inclusive Care for the Elderly (PACE) is a nationally recognized model of care that serves adults aged 55 and older living with chronic medical and/or mental health challenges.
At CHA PACE, our mission is to support our program participants in achieving the highest level of independence 깨 within their communities and preventing nursing home placement. Our high-functioning interdisciplinary team supports this goal by providing fully integrated medical and behavioral health services, community resources, and home-based care in a collaborative and supportive environment.
The PACE Caseworker plays a vital role in supporting participants and families by coordinating services in alignment with the PACE model of care. Working closely with the interdisciplinary team (IDT), the Caseworker helps develop, implement, and monitor participant care and discharge plans that reflect individual goals and team recommendations.
This position also serves as a navigator for new PACE enrollees and their families, helping them understand services, access concrete resources, and transition smoothly into the program. The Caseworker builds empathic, trusting relationships with participants, supports concrete service needs, and documents participant progress in the medical record.
