Overview

Case Manager (RN) - 14212 at Penn Highlands Healthcare. AS A REGISTERED NURSE (RN), CASE MANAGER, you\'ll be responsible for completion of reports such as performance improvement reports, periodic review of departmental polices and procedures. You\'ll also provide Case Management and Social services to specific hospital units, as assigned or per need. You\'ll act as a liaison between members of the Case Management team and representatives from other organizations and providers such as Outpatient Clinics, Home Health Agencies, Long Term Care facilities, Medical Equipment Providers and Area Agencies on Aging.

Responsibilities
  • Complete performance improvement reports and periodic review of departmental policies and procedures.
  • Provide Case Management and Social Services to specific hospital units, as assigned or per need.
  • Act as liaison between the Case Management team and representatives from other organizations and providers (Outpatient Clinics, Home Health Agencies, Long Term Care facilities, Medical Equipment Providers, Area Agencies on Aging).
  • $20,000 sign-on/retention bonus available for qualified candidates.
Qualifications
  • Must have a valid RN License in the commonwealth of Pennsylvania
