Job description
Case Manager, Mental Health Child/Adolescent

The position provides case management services to children aged 4‑21 who have been identified as seriously emotionally disturbed and are at risk for out‑of‑home placement and/or have multi‑agency involvement. Services are delivered in homes, offices, community settings, schools, juvenile detention facilities and residential placements. The primary goal is to keep youth in the least restrictive environment that they can be maintained in, which may sometimes involve residential placement. The case manager monitors progress through on‑site visits, participation in treatment team meetings, and gathering updates from on‑site staff.

Duties and Responsibilities
  • Conduct intake interviews, obtain data and develop diagnoses and treatment plans.
  • Maintain required agency paperwork and filing.
  • Monitor individual progress through family and collateral contacts.
  • Prepare for Family Assessment Planning Team (FAPT) meetings; conduct assessments of needs, identify needed services, select vendors, complete CANS, complete FAPT paperwork and attend the meeting.
  • Attend meetings with Court Services Unit or schools for IEPs, Behavioral Support Plans, discipline meetings, and treatment team meetings.
  • Identify residential placements, complete application packets, secure funding, obtain required signatures, coordinate admission, and attend admission.
  • Monitor individuals in residential placements through treatment team meetings, follow up with staff, follow up with family, conduct residential visits, and complete discharge planning.
  • Participate with Commonwealth Center for Children and Adolescents in discharge planning for all individuals from Planning District 16.
  • Identify and link individuals to resources and services within the community.
  • Meet face‑to‑face with individuals upon discharge.
  • Complete required yearly trainings.
  • Substitute as a Family Assessment Planning Team member.
  • Perform other duties as assigned.
Knowledge, Skills, and Abilities

Thorough knowledge of the diagnosis and treatment of seriously emotionally disturbed children and adolescents, including crisis intervention, behavior management, independent living skills training, supportive counseling, family education, and discharge planning. Knowledge of child and adolescent psychology, physical and intellectual development, family and group dynamics, and individualized services from birth to age eighteen. Ability to manage individual records, maintain confidentiality, and protect individual rights. Familiarity with local community resources, eligibility criteria, intake and termination processes, and generic community resources. Strong oral, written, and interpersonal communication skills, interviewing, observing, recording, and reporting. Skill in coordinating services and negotiating with providers. Ability to work independently under general supervision, and collaboratively with teams and supervisors. Completing training in the High Fidelity Wraparound Model and providing Intensive Care Coordination when requested.

Typical Education, Training and Work Experience

Bachelor's Degree in Behavioral Sciences/Human Services or equivalent education/experience that provides the required knowledge, skills, and abilities. Master’s Degree in Behavioral Sciences/Human Services preferred.

Licensure and Certifications

Valid VA driver’s license required. State professional licensure or eligibility for same preferred.
