Job overview

The Children’s Rapid Response Service have an exciting opportunity as it continues to expand the team. The Children’s Rapid Response Service works collaboratively with the Community Children’s Nursing & Palliative Care Team:It is the team’s primary aim to prevent and reduce hospital attendances, admissions and length of stay for children already known to the Community Children’s Nursing & Palliative Team.

The emphasis of this service is to improve the quality of life for children and families with life limiting, palliative or complex healthcare needs, offering them greater choice around their chosen place of care.

Are you an enthusiastic, dynamic, flexible and motivated experienced Nurse or.teacher Practitioner, with a strong knowledge of Children’s Nursing? Clinical expertise and good communication skills are required as well as excellent time management百分percent and prioritisation skills which are essential for this role. Main duties of the job

We are looking for experienced children’s nurses that have extensive experience in assessment with children with complex health care needs, Ideally the suitable candidate would hold, or be willing to work towards obtaining the independent Non-Medical Prescribing qualification /Txt Assessment Course.

You must have the ability to prioritise your workload using advanced assessment skills with the ability to provide a symptom management plan of care to children/young people during times of acute deterioration or illness.

Job title : Team Leader for Children’s Community Nursing, Palliative Care and Rapid Response

  • ENB 998 (Teaching & Assessing in Clinical Practice) or equivalent
  • Community Children’s Nursing qualification or willingness to undertake
  • Clinical assessment course equivalent or willingness to complete
  • Independent supplementary prescriber or equivalent or willingness to complete
Experience
  • Connections with children with complex healthcare needs/life limiting/ threatening illnesses and end of life
  • Significant experience of working as a Children’s Nurse either RSCN/RN Child
Skills/knowledge
  • Demonstrate knowledge relating to child protection issues and procedures
  • Evidence of assessment skills and use of nursing process and nurse models
  • Demonstrate sound use and application of NMC Code of Conduct and scope of professional care
  • Demonstrate knowledge of current legislative changes affecting NHS and Social Services particularly those affecting primary care
  • Demonstrate knowledge and application of current clinical trends and practices inclusive of evidence based practice in relation to Children’s Community Nursing
  • Ability to teach relatives, patients, clients and staff across all areas of self care and appropriate nursing practice
  • Demonstrate individual responsibility in respect to health and safety
  • Ability to demonstrate written skills that are concise, accurate, relevant and legible
  • Awareness of diversity of the needs of families in relation to culture and religion
